Appendix A: The Fight School Declaration

"With my signature I declare that:

(1) I, of my own free will, wish to enrol as a member of The Fight School for the period from the 1st of January to the 31st of December (YEAR TO BE INSERTED) inclusive.

(2) I understand that The Fight School reserves the right to reject my application or revoke my membership at anytime without any right of appeal.

(3) I am eighteen years of age or over.

(4) I will fully read and comply with the relevant rules and regulations of The Fight School that apply to any specific activity prior to my participation.

(5) I understand that I participate in The Fight School's activities at my own risk, and that these activities are potentially dangerous.

(6) I acknowledge that The Fight School advises that I ensure that I am satisfied that I have adequate personal insurance to cover any such injuries, losses or damages that may occur to me or my property and that my inoculation for Tetanus is up to date during my period of membership.

(7) I will advise the Head of the School of any special medical or dietary issues, that may be relevant to my participation in School activities.

(8) I undertake to release The Fight School, its officers, representatives, and members, from and against all actions, claims, and damages, in respect of any injuries, losses, or damages to me or my property, suffered while taking part in any activities officially attended or organised by The Fight School or as a result of the use or misuse of the official Wayland Fight System as taught and recommended by The Fight School.

(9) I affirm that I will use any training I receive at The Fight School for sports and or leisure activities ONLY and for NO other purpose.

(10) I understand that the "Wayland Fight System" (WFS) is the sole copyright property of Dean C. Wayland, the founder and current Head of the School.

(11) I agree to in NO way whatsoever to pass on training in the Wayland Fight System to any other third party until so qualified and licensed by The Fight School so to do.

(12) I understand that the information given here and subsequent School records maybe stored on computer-readable format and that it will ONLY be used for administration and for NO other purpose, nor will it be supplied to any third party without my prior permission.

(13) I understand this declaration does not remove my rights in law.

(14) I the applicant have read, understood and accept the terms and conditions expressed in this declaration.
